DMS rollout for Morecambe Bay

Lancashire and South Cumbria Integrated Care System (ICS) have been working with the Pharmacy teams at University Hospitals of Morecambe Bay (UHMB) and East Lancashire Hospitals Trust (ELHT) to prepare the spread of a digital platform to support communications between the Hospital Pharmacy teams and Community Pharmacy teams. The platform is called Refer-To-Pharmacy and has been successfully utilised by ELHT since 2015.
One of the key services this will support is the Community Pharmacy Discharge Medicines Service (DMS). ELHT currently send around 1000 referrals per month to community pharmacies using Refer-To-Pharmacy. Other referrals to support transfer of care around medicines are also shared to improve communications generally, including Hospital Admission Notifications to alert Pharmacies so that they may stop dispensing repeat prescriptions in advance while their patient is in hospital.
To prepare for the platform going live in the UHMB footprint, we are inviting you to a Teams session on Tuesday 25th of April to meet the team working on implementation, to provide a demonstration of the system, and provide guidance on interpreting UHMB discharge summaries that will be shared when a referral is sent to your Pharmacy. It is important that at least one Lead Manager / Owner or Lead Clinician from each Pharmacy attends this event. Ideally more than one member of your Pharmacy team should attend as all team members are likely to be involved in the process of accepting and managing referrals.
